Forms and Waivers for RentalsRelease Waiver for ParticipantsEveryone can speed up their registration by filing out our Participant Release of Liability in advance. Each participant must sign. Children under 18 years old are required to have permission of their parent or legal guardian. Participant Release of Liability and Assumption of Risk Agreement Tax Exempt Groups and OrganizationsEligible groups must present a New Jersey Exempt Use Certificate Form ST-4 to waiver Sales Tax charge. The form must be filled out in accordance with the instructions included with the document. For faster check-in, print and complete in advance. New Jersey Exempt Use Certificate Form ST-4 Camping at New Jersey State FacilitiesTo make a reservation, please contact the appropriate park, forest or recreation area office during normal business hours. Reservations can be made up to eleven months in advance of the reservation date in person at the area office; by mail using an overnight application form; or by telephone using a credit card. These facilities can be reserved throughout the year or whenever camping facilities are available for public rental. Reservations for the four, six and eight bunk cabins including cabins accessible for persons with disabilities may be made during normal business hours up to eleven months in advance of the reservation date either in person at the area office; by mail using an overnight application form; or by telephone using a credit card. From the Saturday before Memorial Day through and including Labor Day, cabins may be reserved for seven or fourteen consecutive nights only. From April 1st to the Friday before Memorial Day and from the day after Labor Day through October 31st, cabins may be reserved for a minimum of two consecutive nights to a maximum of fourteen consecutive nights. All other camping reservation procedures apply. Overnight Reservation Application Division of Fish & WildlifeAnyone age 16 and older must have a valid license to fish the fresh waters of New Jersey with handline, rod and line, or longbow and arrow. This includes privately owned lakes and other waters. Fishing licenses must be worn in a conspicuous place on the outer clothing while engaged in fishing, and must be exhibited to law enforcement personnel on request. Purchasing your license is more convenient than ever before. This new I.E.L.S. Integrated Electronic Licensing System site will allow you quick and easy access to fishing licenses, permits, regulations and other important information about recreation in New Jersey. New Jersey Fishing License Information Photograph Release FormVisitors can submit photographs that will be displayed on our website; if judged exceptional by our staff. For each photograph accepted, the person will receive a ten percent discount coupon. We require that visitors complete a release form before the photograph can be published.
